Health Coverage Information Return
Form 1095-B is used to report information on minimum essential coverage (MEC) provided to the primary insured individual by insurers or other providers of minimum essential coverage, such as sponsors of level-funded, or self-funded health coverage (when the sponsor employs less than 50 full time plus full time equivalent employees) . It reports information about health coverage provided during the year to the primary insured and their family members.
